PRN Hours

The Fountain Hill Home Care CNA is a paraprofessional member of the Home Care Team who works under the supervision of a registered nurse and performs various personal care services as necessary to meet the patient’s needs. The Home Care CNA is responsible for observing patients, reporting these observations and documenting observations and care performed, precisely.

The home care CNA will be assigned by a Registered Nurse in a manner that promotes quality, continuity and safety of a patient’s care. Certified as a Nurse Aide I (Certified Nursing Assistant - CNA) and registered with the North Carolina Department of Facility Services Nurse Aide Registry.

Essential Job Functions/Responsibilities

Responsibilities of the home care aide include, but are not limited to, the following:

1. Providing hands-on personal care including:

• Baths/Showers
• Assistance with Ambulation
• Oral hygiene
• Shampoos
• Changing bed linen
• Assisting patients with dressing and undressing
• Skin care to prevent breakdown
• Assisting the patient with toileting
• Keeping patient’s living area clean and orderly, as appropriate

1. Planning and preparing nutritious meals, when ordered.
2. Assisting in feeding the patient, if necessary.
3. Taking and recording oral and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency).
4. Assisting in ambulation and exercise according to the plan of care.
5. Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
6. Performing simple procedures as an extension of nursing services as ordered (with appropriate completed/demonstrated skills competency)
7. Assisting patient in the self-administration of medication.
8. Doing patient’s laundry, as appropriate.
9. Meeting safety needs of patients and using equipment safely and properly (foot stools,
   side rails, etc.).
10. Reporting on patient’s condition and significant changes to the assigned nurse.
11. Adhering to the Organization's documentation and care procedures and standards of personal and professional conduct.
12. Work a flexible schedule both as Home Care Caregiver and Concierge CNA. Ability to work some weekends as needed.
13. Meet the Physical parts of the “Extra Hand” position that require flexible shifts sitting, walking, standing, pushing residents in wheelchairs, and lifting.

Position Qualifications

1. Certified as a Nurse Aide I (CNA) and registered with the North Carolina Department of Facility Services Nurse Aide Registry.
2. Meets the training requirements and successfully completes:

• A training and competency evaluation program that meets §484.80 (b) and (c); or
• A competency evaluation that meets the requirements of §484.80 (c); or
• A nurse aide training and competency evaluation approved by the state as meeting the requirements of §483.151 through §483.154 and is currently in good standing on the state nurse aide registry; or
• The requirements of a state licensure program that meets the provisions of §484.80

1. At least 18 years of age.
2. Ability to read and follow written instructions and document care given.
3. Self-directing with the ability to work with little direct supervision.
4. Empathy for the needs of the ill, injured, frail and the impaired.
5. Possess and maintains current CPR certification.
6. Demonstrates tact, patience and good personal hygiene.
7. Valid Driver’s License.
8. Ability to Multi Task with adaptability and flexibility
9. Ability to stand, climb steps and walk for lengths of time.
10. Ability to schedule tasks using good time management skills.
11. Perform Duties as both “An Extra Hand” Services and Home Care CNA
12. Bilingual a plus, but not required

Job Limitations

The home care aide will not function in any manner viewed as the practice of nursing according to the state's Nurse Practice Act. Specifically, the home care aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge and skill of a nurse, such as sterile techniques.
