Job Summary
As an on-site program manager/case-manager in a child group home of three. Aquarium Ke Program /case Manager will be responsible in overseeing operations ensuring the well-being and safety of the children.
Case Manager/Program Manager Duties
- Implementing and monitoring programs and activities
- Develop and implement individualized care plans for children based on assessments, needs, and goals.
- coordinating with external stakeholders such as social workers or therapists, maintaining compliance with regulations, handling emergencies when needed
- providing support and guidance to both the children and the provider/Manager.
- Developing and maintaining relationships with the children's families or guardians to promote family involvement and reunification when possible.
- Collaborating with other professionals to meet the children's needs.
- Advocating for children’s rights and best interests.
- The program manager will ensure each child's or youth's individualized treatment, and care plan is implemented effectively. They will monitor progress, adjust approaches as needed, and collaborate with relevant professionals to optimize care outcomes.
- Strong management and leadership abilities with experience supervising teams in social or behavioral health settings.
Qualifications of a Program /Case Manager.
- Bachelor's degree in social work, psychology, counseling, or a related field .Associate degree would work
- Two to three years of previous experience in case management, program management with children/youth with developmental disabilities
- Knowledge of child development, trauma-informed care, and best practices in working with children and families.
- Strong communication, interpersonal, and organizational skills.
- Ability to collaborate effectively with a multidisciplinary team and external partners.
- Knowledge of relevant laws, regulations, and standards related to child welfare and residential care.
- Crisis intervention skills and the ability to manage challenging situations professionally and compassionately.
- Familiar with Therap software or open to learning more about the software will be an added advantage.
