Registered Nurse – Community Living Center (CLC)
Position Overview
Compensation: 50.00/hr plus $5.09 for Health and Wellness
We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality care in a Community Living Center (CLC) setting. This role focuses on short-term rehabilitation, skilled nursing care, and long-term support for Veterans transitioning from acute care to home or community environments.
Primary Responsibilities
Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, IV therapy, tube feedings, and wound care
- Provide tracheostomy care, catheter management, and chronic disease support
- Assist patients with activities of daily living (ADLs)
- Collaborate with interdisciplinary teams to support recovery and discharge planning
- Document care accurately in the electronic health record (EHR)
- Respond to medical emergencies and changes in patient condition
Work Environment
- Post-acute rehab, dementia care, hospice, and skilled nursing units
- Patient population includes rehabilitation, respiratory, cognitive impairment, and end-of-life care
Minimum Qualifications
Minimum Qualifications
- Graduate of an accredited nursing program (ACEN or CCNE)
- Active RN license (Ohio or compact state)
- Current BLS certification (ACLS required for ICU/ED)
- Minimum 2 years of recent clinical experience
- Strong English communication and documentation skills
Additional Requirements
- Ability to work 12-hour shifts, including nights, weekends, and holidays
- Completion of VA-required training (safety, HIPAA, EHR, etc.)
- Compliance with credentialing and privileging standards
- Ability to float between units as needed
