About The Role
- The ECM Clinical Supervisor (Clinical Consultant – Enhanced Care Management) provides clinical oversight, supervision, quality assurance, and CalAIM-compliant care review for APF’s Enhanced Care Management (ECM) program.

What You'll Do
- Clinical Oversight & Supervision
- Provide routine clinical supervision to ECM Care Managers & CHWs
- Review & sign all ECM care plans (initial annual updates)
- Conduct monthly chart reviews for MCP and CalAIM compliance
- Guide staff on risk mitigation, crisis intervention, and escalation pathways
- Support complex case consultations
- Lead continuous quality improvement and documentation audits
- Support MCP audits and quality reviews
- Ensure all services meet ECM contract and state guidelines
- Deliver ongoing clinical training and upskilling
- Represent APF in MCP meetings and clinical contract reviews
- Build collaborative relationships with hospitals, FQHCs, BH providers, and housing partners
- Provide short-term assessments or case conferencing during high-volume periods
- ~32 hours/month (approx. 8 hours/week)
- Base stipend: $2,000/month
- Plus $50 per care plan review and signature
- Estimated annual compensation: ~$30,250 (equivalent to ~$79/hour)
Qualifications
- Bilingual (Spanish/English or other threshold language)
- Experience with CalAIM ECM and/or Community Supports
- Prior supervision of interdisciplinary or care management teams
- 100% timely review & signature of ECM care plans
- ≥90% documentation and audit compliance
- Strong collaboration between ECM teams, CHWs, and MCP partners
- Effective management of high-risk and escalated cases
- A well-trained, culturally responsive care team
