Pay Rate: $25.86

Job Summary: Key Job Responsibilities
Performs health information activities necessary to organize, maintain and use electronic and paper patient
health records. Depending on area assigned, may specialize in one or more clerical functions, such as
qualitative/ quantitative physician documentation review; and tumor registry support and follow-up. If assigned
to the Torrance Records Retention Center, receives and maintains archived records, responds to requests for
information/ records and serves as customer service liaison with main campus and satellite locations. Queries
multiple electronic record and tracking systems. Understands chart organization and content, CSMC and
external requirements related to chart documentation and privacy.
Primary Duties and Responsibilities
• Locate and request patient records/ information needed for Registrars to complete CNext abstracting.
• Research patient activity to determine applicability to the follow up monitoring/ reporting processes.
• Review case finding reports to select cases for inclusion in the Tumor Registry.
• Generate patient follow up letters for distribution to patients/ family/ contacts within the registry to solicit
responses with required follow up information.
• Receive and review follow up letter responses.
• Call patient family/ contacts to solicit required follow up information.
• Enter data received from follow up communications into the Cnext follow up module.
• Maintain follow up timeliness and percentages as required for accreditation.
• Understands the type and composition of patient information necessary for tumor registry specific abstracting
that adheres with Association (ACOS), Organization, (CSMC), and State of CA and Federal guidelines.
• Complete special projects pertaining to service unit assigned.
REQUIREMENTS:
•HS DIPLOMA/GED
•2-3 YEARS OF EXPERIENCE IN HEALTH CARE SETTING OR RELATED FIELD REQUIRED
