Job Description:
Canaide, Inc., doing business as ApprioHealth is currently seeking a highly professional Patient Advocate in Orlando, FL. He/ She will act as the initial contact person to pursue the application processes of the various programs including but not limited to completing the application, following up to obtain all necessary documentation, & maintaining ongoing communication with the patient/guarantors, case managers and or doctors.
Responsibilities:
- Will travel to various locations within the Orlando area including: Kissimmee, Orlando, East Orlando, Winter Park, Altamonte Springs, Apopka, Celebration, Winter Garden, Lake Nona and Minneola.
- Visit patients in the hospital & bedside to pre-screen for qualifying criteria of various Federal & State Medicaid/Social Security Administration (SSA) & County / HCRA programs
- Maintaining ongoing communication with various agencies throughout the application process
- Documentation/other paperwork as well as computerized record keeping
- Assess cases appropriately & utilize the most appropriate community resources to provide efficient client services
- Prepare clear & concise reports, correspondence & other written materials
- Use initiative & independent judgment within established procedural guidelines, organize own work, set priorities, & meet critical deadlines
- Establish & maintain effective working relationships with those contacted in the course of work
- Communicate exceptionally well orally, in writing, & over the telephone
- Process high volumes of cases within a specified amount of time as determined by the Client, State of Florida and the Management Team
Educational Requirement: High School Diploma or GED
Required Experience: 1 to 3 years of experience
Requirement Knowledge:
- Must be able to respond within 30 minutes to any same day request and be available for a rotation system to ensure fair distribution across all facilities.
- Principles & practices of social work & case management techniques used in the Medicaid program
- Federal regulations & state laws applicable to the Medicaid program
- Advanced interviewing skills
- Computer applications including proficiency in Microsoft Access, Outlook, Excel, & Word
- Record keeping principles & practices including spelling, grammar, & punctuation
- Techniques for dealing with a variety of individuals from various socioeconomic, ethnic & cultural backgrounds
- Programs & eligibility requirements for the Social Security Administration, State Medicaid, County Social Services, & other relevant programs
