Job Summary
The Residential Care Manager is responsible for overseeing and coordinating resident care services within the skilled nursing facility. This role ensures high-quality, person-centered care by managing resident care plans, supervising direct care staff, and collaborating with interdisciplinary teams to enhance the well-being of residents. The Residential Care Manager will uphold compliance with state and federal regulations, support staff development, and promote a culture of compassion and excellence in resident care.
Key Responsibilities
Resident Care Management:
Oversee the development, implementation, and evaluation of individualized resident care plans.
Conduct regular assessments to ensure residents' needs are met in accordance with regulatory requirements.
Coordinate with nursing, therapy, and social services teams to provide comprehensive care.
Act as a liaison between residents, families, and healthcare providers to ensure effective communication and continuity of care.
Staff Supervision & Development:
Supervise and support direct care staff, ensuring adherence to facility policies and best practices.
Provide coaching, training, and performance evaluations to enhance staff competency and engagement.
Assist in staff scheduling and coverage to maintain adequate staffing levels for quality care delivery.
Compliance & Quality Assurance:
Ensure compliance with all federal, state, and local regulations governing skilled nursing facilities.
Participate in audits, inspections, and quality improvement initiatives.
Monitor and address resident care concerns, incidents, and grievances promptly.
Resident & Family Engagement:
Foster a resident-centered approach to care, emphasizing dignity, choice, and quality of life.
Educate families on care plans, facility policies, and available resources.
Promote resident participation in activities and wellness programs.
Qualifications & Requirements:
Education: Associate's or Bachelor's degree in Nursing (RN or LPN), Social Work, Healthcare Administration, or a related field preferred.
Experience: Minimum of 2 years of experience in long-term care, assisted living, or skilled nursing, with prior leadership experience preferred.
Licensure/Certification: Active RN or LPN license (if applicable) in the state of employment preferred; CPR certification required.
Skills:
Strong leadership and organizational abilities.
Excellent communication and interpersonal skills.
Knowledge of state and federal regulations governing skilled nursing facilities.
Proficiency in electronic health records (EHR) and care planning systems.
Work Environment & Physical Requirements:
Ability to work in a fast-paced environment with residents who have varying levels of physical and cognitive needs.
Requires standing, walking, bending, and lifting up to 50 lbs. as needed.
May require evening, weekend, or holiday shifts based on facility needs.
