Job Summary

The Residential Care Manager is responsible for overseeing and coordinating resident care services within the skilled nursing facility. This role ensures high-quality, person-centered care by managing resident care plans, supervising direct care staff, and collaborating with interdisciplinary teams to enhance the well-being of residents. The Residential Care Manager will uphold compliance with state and federal regulations, support staff development, and promote a culture of compassion and excellence in resident care.

Key Responsibilities

Resident Care Management:

  • Oversee the development, implementation, and evaluation of individualized resident care plans.

  • Conduct regular assessments to ensure residents' needs are met in accordance with regulatory requirements.

  • Coordinate with nursing, therapy, and social services teams to provide comprehensive care.

  • Act as a liaison between residents, families, and healthcare providers to ensure effective communication and continuity of care.

Staff Supervision & Development:

  • Supervise and support direct care staff, ensuring adherence to facility policies and best practices.

  • Provide coaching, training, and performance evaluations to enhance staff competency and engagement.

  • Assist in staff scheduling and coverage to maintain adequate staffing levels for quality care delivery.

Compliance & Quality Assurance:

  • Ensure compliance with all federal, state, and local regulations governing skilled nursing facilities.

  • Participate in audits, inspections, and quality improvement initiatives.

  • Monitor and address resident care concerns, incidents, and grievances promptly.

Resident & Family Engagement:

  • Foster a resident-centered approach to care, emphasizing dignity, choice, and quality of life.

  • Educate families on care plans, facility policies, and available resources.

  • Promote resident participation in activities and wellness programs.

Qualifications & Requirements:

  • Education: Associate's or Bachelor's degree in Nursing (RN or LPN), Social Work, Healthcare Administration, or a related field preferred.

  • Experience: Minimum of 2 years of experience in long-term care, assisted living, or skilled nursing, with prior leadership experience preferred.

  • Licensure/Certification: Active RN or LPN license (if applicable) in the state of employment preferred; CPR certification required.

  • Skills:

    • Strong leadership and organizational abilities.

    • Excellent communication and interpersonal skills.

    • Knowledge of state and federal regulations governing skilled nursing facilities.

    • Proficiency in electronic health records (EHR) and care planning systems.

Work Environment & Physical Requirements:

  • Ability to work in a fast-paced environment with residents who have varying levels of physical and cognitive needs.

  • Requires standing, walking, bending, and lifting up to 50 lbs. as needed.

  • May require evening, weekend, or holiday shifts based on facility needs.
