Overview

The stroke coordinator is responsible for coordinating and implementing all stroke services and activities for the assigned community. He/she functions as a liaison between the LIPs, nursing staff, allied healthcare professionals, patients and their families; coordinates and facilitates the care for the stroke patient throughout the continuum; works closely with LIPs and nursing to ensure clinical practice guidelines are followed. The stroke coordinator participates in program development as it relates to the hospital and community. This includes, but is not limited to staff education, community education, and marketing. He/she is responsible, in coordination with the stroke program medical director, for planning, developing and implementing stroke center initiatives. Will maintain quality standards for responsive service including data management andperformance improvement for key quality indicators. He/she will take a leadership role in development and implementation of outcomes evaluation as guided by DNV standards and evidence-based guidelines.

Responsibilities

  • Assists with the coordination of multidisciplinary care for stroke patients in collaboration with LIPs and nursing.
  • Provides clinical resources and consultation to staff regarding stroke patients with clinically complex needs.
  • Works with department managers and directors to implement formal and informal nurse and hospital education and training regarding stroke care.
  • Assists with development and coordination of community education for healthcare professionals and the lay community.
  • Completes chart reviews to ensure compliance with stroke accreditation guidelines.
  • Develops and implements initiatives to improve care in coordination with the Stroke Program Director.
  • Works with the Stroke Medical Director in the promotion and marketing of the stroke program.
  • Attends and participates in local quality management system meetings.
  • Attends and participates in monthly ARH System Stroke Coordinator meetings.
  • Attends and participates in quarterly Norton/UK Stroke Program Coordinator meetings.
  • Represents and promotes stroke services to hospital staff, volunteers, patients, and visitors through a positive attitude and effective communication skills.
  • Exhibits professional conduct and demeanor during all interactions.
  • Acts as a liaison between LIPs, nursing departments, and ancillary staff on behalf of patients and their families.
  • Ensures that stroke care is consistent with current evidence-based best practices.
  • Leads and/or assists with coordination of performance improvement program initiatives, including outcome evaluations and action planning as necessary.
  • Communicates with leadership regarding any issues involving service, personnel, or patients.
  • Leads local stroke committee meetings (quarterly at minimum) and provides program performance reports at facility leadership meetings.
  • Ensures submission of quality metrics to Norton/UK Stroke Care Network on or before the quarterly due date.
  • Maintains current certification in the NIHSS and other required regulatory components of the position.
  • Completes a minimum of eight (8) hours of stroke-specific continuing education annually.

Qualifications

Education

  • Associate Degree in Nursing required.
  • Bachelor's Degree in Nursing preferred.

Experience

  • One (1) to three (3) years of experience in an Emergency Department, ICU, or acute care setting required.
  • One (1) to two (2) years of experience in process improvement or leadership preferred.

Required Skills, Knowledge & Abilities

  • Knowledge of stroke care standards, accreditation requirements, and evidence-based practice.
  • Ability to coordinate multidisciplinary patient care across the continuum of care.
  • Strong leadership, communication, and collaboration skills.
  • Ability to conduct chart reviews, analyze quality metrics, and identify opportunities for process improvement.
  • Ability to develop and implement performance improvement initiatives.
  • Ability to provide education and training to healthcare professionals and community members.
  • Strong organizational and project management skills.
  • Ability to build effective relationships with physicians, nursing staff, ancillary departments, patients, and families.
  • Ability to communicate effectively with leadership regarding program performance and patient care issues.

Licenses & Certifications

  • Current Registered Nurse (RN) license required.
  • Current NIH Stroke Scale (NIHSS) certification required or ability to obtain and maintain certification.
  • Minimum of eight (8) hours of annual stroke-specific continuing education required.
