Maintain the training and education needed to provide patient-centric pharmacy services. Retail: Will obtain and maintain the training and education needed to provide comprehensive pharmacy services, including but not limited to naloxone, immunization certification, medication therapy management, state and federal laws and regulations. |
Responsibilities:
- Responsible for properly assessing patients' pharmaceutical needs and providing pharmaceutical care.
- Responsible for the accurate and proper compounding and dispensing of prescribed medications and solutions.
- Responsible for building and maintaining partnerships with patients and other healthcare providers to help build a strong multidisciplinary healthcare team that works to improve patient outcomes.
- Responsible for properly counseling patients/patients' families, and appropriate health care professionals regarding drug therapy.
- Responsible for daily pharmacy workflow and pharmacy operations in the absence of the pharmacist-in-charge or designee.
Activities:
- Supervises and performs the functions involved in the accurate and efficient dispensing of prescriptions.
- Reviews, fills, and verifies medication orders for accuracy and completeness for dispensing.
- Monitors patient's drug therapy to detect/prevent drug interactions, contraindications, and other adverse reactions.
- Screen all new medication orders for appropriate dose, drug interactions, allergies, and contraindications prior to dispensing medication.
- Counsel patients/patients' families, and appropriate health care professionals on all new medication orders.
- Make appropriate interventions to improve patient care that is based on current guideline-directed therapy.
- Makes appropriate interventions to improve patient care.
- Uses and is proficient in the operation of the pharmacy computer system.
- Compounds and dispenses medications and other pharmaceutical agents/products.
- Obtains pertinent patient medication/medical information.
- Provides current and accurate drug information to patients and health care professionals.
- Communicates effectively with patients, health care professionals, and coworkers.
- Serves on hospital and system committees as assigned.
- Completes and properly documents medication therapy management interventions.
- Ensures compliance with company and legal policies, procedures, and regulations.
- Ensures confidentiality of information, documentation, and records.
- Performs other related duties as assigned.
