Responsibilities:

a.    Maintain required credentials;
    
b.    Deliver, delegate, monitor, and supervise medically related services to individuals in  community living and day program  settings;

c.   Supervise Staff Nurse(s);

d.    Supervise all UAP/CNA/CMT staff in the performance of delegated nursing tasks;

e.    Contribute to the development of Individual Plans (IPs);

g.    Conduct staff training; and

h.    Other ongoing responsibilities.

Job Summary:

a.    Maintain required credentials

1.  Keep nursing license current and give a copy to CLSS Director;

2.  Obtain DDA Certification from DDA Regional Nurse; 

3.  Attend DDA Quarterly Nursing meetings at least 2 to 3 times a year to keep DDA Certification updated; and

4.  Attend and satisfactorily complete all required trainings.

b.    Deliver, delegate, monitor, and supervise medically related services to individuals in community living and day program settings
        

  1. Complete the Initial Nursing Assessment;  
  2. Develop, monitor, and revise the Nursing Care (NCP) for each client;    
  3. Complete Post Hospitalization Assessments as needed;
  4. Complete or delegate and review all 45 Day Nursing Assessments;
  5. Initiate and/or delegate obtaining PMOF’s and medications;
  6. Initiate and/or delegate NCP approaches and other medical/nursing related duties to nursing staff and all UAP/CNA/CMT staff as appropriate; 
  7. Complete and/or review nursing screening tools, (i.e., Health Risk Screening Tool, Choking Risk Screening Tool, etc.)
  8. Monitor the medical status of individuals with  nursing care plans and inform appropriate Program Coordinators and/or Program Directors of any potential dangers or health constraints; 
  9. Assist with obtaining appropriate health information (laboratory results, medical appointment forms, doctor’s orders, diagnostic test results) for the client’s healthcare record;
  10.  Review resident/trainee injury reports; and
  11. Be readily available by pager and/or cell phone to address all medical/nursing concerns of a client as they arise.

    
  c. Supervise Staff Nurse(s)

  1. Assign nursing functions to RN/LPNs, supervise their performance of nursing tasks and ensure that tasks are performed appropriately in compliance with the Maryland Nurse Practice Act;
  2. Orient new staff to medical/nursing related job duties;
  3. Complete staff evaluations as appropriate;
  4.  Review time sheet(s);
  5.  Delegate job duties as needed;
  6.  Coach and provide feedback to staff regarding job performance;
  7.  Recommend corrective and/or disciplinary action as needed to Program Directors;    
  8.  Monitor staff injury reports; and
  9.   Assure adherence to agency program and personnel policy and procedure.

d.    Supervise all UAP/ CNA/CMT staff in the performance of delegated nursing tasks

  1.  Supervise the UAP/CNA/CMT to ensure competency in performing delegated nursing tasks and documenting the competency and supervision;
  2.  Delegate nursing functions to UAP/CNA/CMT based on nursing care plan;
  3.  Supervise the performance of delegated nursing tasks;
  4.   Recommend corrective and/or disciplinary action as needed to Program Directors;    
  5.   Monitor staff injury reports; and
  6.  Assure adherence to and compliance with the COMAR regulations, agency program and personnel policy and procedure.

e.    Contribute to the development of the Individual Plan (IP)

      1.    Develop and recommend nursing goals;
      2.    Recommend medical/nursing related staff trainings;
      3.    Recommend special dietary needs; and      
      4.    Consult with Program Coordinators as needed.
 
f.    Conduct staff training

  1. Provide training to the UAP/CNA/CMT as required by regulatory bodies;  
  2. Provide training to the UAP/CNA/CMT for all health care procedures/activities;
  3. Review the client’s nursing care plan and medical condition with the UAP/CNA/CMT staff;
  4. Conduct Dining and Choking Training; and
  5. Conduct other training’s as assigned by the CLSS Director.

g.    Other ongoing responsibilities

  1.  Document all nursing interventions, including telephone interventions, in accordance with nursing standards of practice and agency procedure;
  2.  Prepare other appropriate reports and documentation as directed hazards;
  3.  Promote and protect the rights and welfare of individuals served by the agency;
  4.  Be a liaison between family members and health care professionals;
  5.  Consult with Program Coordinators regarding all medically related reportable incidents and assist with initial and investigative reports; 
  6.  Promote and maintain cordial, effective, and appropriate communication with others involved in resident’s welfare;
  7.  Maintain a working knowledge of the agency’s policies and procedures and comply with the same;
  8.  Develop and implement health care policies as well as assist with appropriate system processes within the agency to comply with regulatory bodies both internal and external;
  9.  Be an active member of the Quality Assurance Advisory Committee;
  10.  Be an active member of the Community Living Support Services and Day Program Management Teams;
  11. Participate in committees within the agency related to nursing care, health, and safety;
  12. Attend and participate in other required staff meetings;
  13. Attend other trainings as assigned by the CLSS Director; and
  14. Any other duties as directed by the CLSS Director.

Supervision Exercised:
The Agency Nurse will be responsible for the overall supervision of all assigned nursing staff. 

Supervision Received:

The Agency Nurse will be supervised by the Community Living Support Services Director.

Qualifications:

Qualifications:

a.   Must be a Registered Nurse with a current license and registered with the Maryland Board of Nursing;

b.   Prefer at least two years of supervisory experience;

c.   Must possess a valid driver’s license and maintain an acceptable driving record; and

d.   Must meet essential physical requirements of lifting and transferring individuals.
