We are urgently hiring an RN Care Manager for a hybrid position based in Seattle, WA. This unique opportunity offers the flexibility of working remotely three days a week, with two days on-site at a local clinic. All necessary remote equipment will be provided.

Position Title: RN Care Manager

Location: Seattle, WA (Hybrid - 2 days onsite, 3 days remote)

Start Date: ASAP

Contract Type: 5x8hr, Monday to Friday (8:00 AM - 4:00 PM)

Experience Required:

• Minimum 1 year in an Acute Hospital, Long Term Acute Care, Rehab, or Skilled Nursing setting
• EPIC charting experience required First-Time Travelers Accepted: Yes Floating: As needed within skillset On-Call Requirement: None Weekends Required: No Local & Non-Local Candidates: Both accepted
  
  

Position Highlights:

• Focus on transitions of care, including care coordination and medication reconciliation
• Aims to reduce emergency department visits and hospital readmissions
• Manages all inpatient discharges (excluding labor & delivery, and oncology follow-up)
• Reviews all ED visits with a special focus on patients with chronic conditions

• Candidates must have reliable personal transportation (public transportation not acceptable)
• Outpatient clinic setting for on-site days
• All remote equipment will be issued by the clinic
