Social Worker/Counselor

Job Summary
We are seeking a compassionate and dedicated Social Worker/ Conselor to join our team. The ideal candidate will be responsible for providing support and assistance to individuals and families facing various challenges. This role involves conducting assessments, developing treatment plans, and facilitating group therapy sessions. The Social Worker / Counselor will work closely with patients to promote their well-being and help them navigate through difficult situations.

Responsibilities

• Conduct comprehensive patient assessments to determine needs and develop individualized treatment plans.
• Provide direct patient care, including individual and group counseling sessions.
• Facilitate group therapy sessions to promote social interaction and emotional healing among participants.
• Maintain accurate medical documentation in compliance with legal and ethical standards.
• Manage cases effectively, coordinating with healthcare providers, families, and community resources.
• Utilize knowledge of medical terminology to communicate effectively with healthcare professionals.
• Implement behavioral therapy techniques to support patients in achieving their therapeutic goals.
• Work with geriatric populations, addressing their unique social, emotional, and health-related needs.
• Advocate for patients’ rights and access to necessary services within the healthcare system.

Requirements

• A Master's degree in Social Work, Counseling or a related field is required; licensure as a LSW, LPC, or higher .
• Experience in behavioral health settings is highly desirable.
• Strong skills in patient assessment and case management are essential.
• Familiarity with medical documentation practices is necessary for maintaining accurate records.
• Proficiency in individual counseling.
• Experience with working with adolensense a plus.
• Excellent communication skills, both verbal and written, are crucial for effective interaction with client and colleagues.
• A commitment to providing compassionate care while maintaining professional boundaries is essential.
