Community Recovery Specialist- PRN
Slatington, PA
Position Summary
CRS is primarily responsible for providing daily support for residents in relation to daily living, self-care, support as it relates to treatment being received. Model behavior that is consistent with treatment outcomes. Facilitate individual and group counseling. A commitment to the RHD values should be demonstrated as job duties are performed.
Essential Duties and Functions
- Assisting residents in skill building with a focus on independent living; employment, education, money management, medication management, transportation, meal preparation, shopping for household goods, site cleanliness, housing searches, roommate matching and social skills development.
- Assisting residents with appointments, following up with medical care and managing medications.
- Managing a caseload of 1-3 residents with the goal of moving them onto a lesser level of care in one year’s time.
- Focusing on Psychiatric Rehabilitation principles while implementing the phase system, service planning, skill building, goal planning and community integration.
- Keeping detailed documentation of progress through daily notes, service- planning, goal documentation and medication monitoring/MAR.
- Attending weekly team meetings, engaging in supervision with site lead (minimum once a month), attending monthly all staff meetings and meeting with psychiatric rehabilitation specialist as needed to focus on any barriers in service planning and skill building with residents.
- Ensuring compliance with all programmatic and corporate procedures and protocols as they apply to staff, residents, and the program.
- Informing management of any kind and all incidents which may affect the welfare of staff, residents, or the program immediately.
- Conducting oneself in a manner that exemplifies professionalism, fairness and compassion at all times.
- CRS should have a minimum of a high school diploma with three years experience in a related field.
- All other duties as directed.
Qualifications
- Bachelor’s degree preferred, High School Diploma required.
- Minimum of three years’ experience working directly with individuals diagnosed with mental health or IDD.
- Current and valid driver’s license with ability to travel to programs and the Corporate Office as needed.
- Criminal record clearances, as dictated by the assigned programs
Lifting Requirements
- Light: exerting up to 20 pounds of force frequently, and/or negligible amount of force constantly to move objects. The use of arm and/or leg control requires force greater than sedentary, but worker still sits majority of time.
