Position Summary

We are seeking a highly motivated and empathetic Care Navigator with a passion for improving the healthcare journey. This role will focus on member engagement and enhancing the end-to-end member experience. You will support patients by guiding them through the medication process and providing ongoing support. The ideal candidate brings a strong background in healthcare, care coordination, or patient advocacy, and is comfortable engaging with members to build trust and improve outcomes.


Key Responsibilities

  • Manage patient enrollments and guide members through onboarding with empathy and clarity
  • Educate members about medication access, coverage, and delivery processes
  • Follow up with physicians and pharmacies to obtain and update prescriptions (script chasing)
  • Coordinate medication delivery and address issues or delays to ensure seamless access
  • Continuously evaluate and improve the member experience throughout the care journey
  • Document member interactions and maintain accurate case records in the system
  • Collaborate with internal teams to align engagement strategies and patient satisfaction goals


Qualifications

  • 3 years of experience in healthcare roles such as care management, patient navigation, or pharmacy services
  • Background in treatment adherence support, case coordination, or member-facing roles is preferred
  • Excellent communication and interpersonal skills with a member-first mindset
  • Detail-oriented with strong organizational and time management abilities
  • Comfortable working in a fast-paced, tech-enabled environment
  • Familiarity with EHR systems, pharmacy processes, and care navigation tools is a plus
