Position Summary:
The Community Intervention Monitor (CIM) plays a critical role in supporting the success and well-being of residents at Apex Transitional Housing. This position is responsible for monitoring, documenting, and supporting the implementation of community-based interventions and programs aimed at improving safety, stability, and social outcomes for individuals and families in transitional housing.
Key Responsibilities:
Monitor the effectiveness of community programs and resident support initiatives
Serve as a liaison between residents, staff, and external service providers
Identify and report potential risks, conflicts, or needs within the community
Support crisis intervention and de-escalation efforts as needed
Conduct regular walkthroughs and check-ins to ensure a safe and supportive environment
Track and document resident progress and community engagement outcomes
Assist in the coordination of community meetings, workshops, and conflict-resolution sessions
Qualifications:
Experience in social services, community outreach, or conflict mediation
Strong communication and interpersonal skills
Ability to remain calm and professional in high-pressure situations
Commitment to trauma-informed and culturally competent care
Background in behavioral health, criminal justice, or community safety (preferred
