Lead Nurse Practitioner (Hormone Health &
Performance)
– Fee based pay with Portfolio
Compensation: $60,000 - $180,000 per year (Uncapped Performance Model)

APEX Rejuvenation & Performance is a high-growth Hormone Health organization scaling rapidly across North Carolina, Georgia, and Florida. We are not a standard "Minute Clinic." We are building a multi-state performance medicine empire.
We are looking for a Lead Nurse Practitioner who has a business mindset. We do not pay a flat hourly wage that caps your potential. Instead, we offer a "Portfolio Model" that pays you a royalty on every patient you retain, creating true recurring income.
Compensation & Incentives
Start Bonuses (Cash Upfront)
We invest in our providers while you build your book of business.
* $1,500 Signing Bonus: Paid upon contract signing and credentialing completion.
* $1,000 "Scaling" Bonus: Paid at Month 3 to reward successful launch momentum.
The Portfolio Model (Recurring Revenue)
You earn Active Income (Visit Fees) Passive Income (Retention Bonuses).
* New Patient Consult (GFE): $60.00 per visit
* Follow-Up Visit: $25.00 per visit
* The Wealth Builder: $10.00 per month / per active patient (Paid monthly as long as the patient is active).
The Roadmap (How You Get to $15k /Month)
Phase 1: The Launch (Months 1–3)
Focus: Building the base & Cash Flow.
* Incentives: $1,500 Signing Bonus $1,000 Month 3 Bonus.
* Volume: ~40 New Patients 20 Follow-ups.
* Est. Pay: $4,000 – $5,500 / month (Part-Time Hours).
Phase 2: The Momentum (Mon
ths 6–12)Focus: Consistent Growth.
• Volume: 250 Active Members (Conservative Target).
• Retention Bonus: $2,500/month (Guaranteed Base).
• Visit Fees: ~$4,500/month.
• Est. Pay: $7,000 – $8,000 / month (Solid Secondary Income).
Phase 3: The Scale (Year
1.5 - 2)Focus: Managing the Book of Business.
• Volume: 500 Active Members across 3 states.
• Retention Bonus: $5,000 /month (Passive Base Pay).
• Visit Fees: ~$8,000/month.
• Est. Pay: $13,000 – $15,000 / month.
The Role
* Clinical: Conduct Good Faith Exams (GFEs) via Telehealth for Men’s Hormone Therapy (TRT), Peptide Therapy, and Medical Weight Loss (GLP-1s)
* Protocol Design: Review labs and create personalized treatment plans (Support provided by Medical Director).
* Telehealth Expansion: Manage patient panels across NC, and expand into GA/FL/TN as we scale.
* Leadership: As the Lead NP, you will have the opportunity to oversee junior providers as the clinic grows.
Requirements
* Active Nurse Practitioner License in North Carolina (Required).
* Multi-State Licenses: Active licenses in Georgia, Florida, Tennessee, or Virginia are a massive plus (Higher signing priority).
* Active DEA Registration.
* Business Mindset: You understand that patient retention = higher income. You treat your patient panel like your own business.
* Experience in TRT, HRT, or Weight Loss preferred (Training provided for the right candidate).
