As a Home Care Occupational Therapist, you will focus on enhancing clients' independence and quality of life through personalized therapy plans.
Key Responsibilities:
- Conduct in-home assessments to evaluate clients' daily living needs and challenges.
- Develop and implement customized occupational therapy plans to address clients' specific goals.
- Provide therapeutic interventions to improve clients' ability to perform daily activities.
- Collaborate with clients, families, and healthcare professionals to ensure a holistic approach to care.
- Educate clients and their families on adaptive techniques and equipment for increased independence.
- Maintain accurate and detailed documentation of assessments, interventions, and progress.
- Degree in Occupational Therapy from an accredited program.
- State licensure as an Occupational Therapist.
- Strong clinical and communication skills.
- Ability to adapt therapy plans to diverse client needs.
- Compassionate and patient-centered approach to care.
- Valid Driver’s License and/or reliable transportation.
- Valid Liability Insurance
