What You’ll Do

• Monitor and help improve clinical quality measures and patient outcomes
• Identify gaps in care and work with providers and clinical teams to address them
• Collaborate with insurance plans regarding quality initiatives, performance measures, and patient-care needs
• Review patient information and clinical data to identify opportunities for follow-up and intervention
• Support preventive care, screenings, chronic disease management, and other quality initiatives
• Assist providers with clinical needs related to value-based care and quality improvement
• Coordinate patient outreach and follow-up as needed
• Help improve workflows, documentation, and processes that support quality goals
• Serve as a clinical resource to providers and staff
• Work collaboratively across departments to improve the overall patient experience and quality of care

What We’re Looking For

• Current RN license
• Clinical nursing experience, preferably in primary care, family medicine, ambulatory care, care management, or a similar setting
• Experience with quality improvement, population health, care management, HEDIS/quality measures, or value-based care is highly desirable
• Comfortable reviewing clinical data and identifying gaps in patient care
• Strong communication and organizational skills
• Ability to work collaboratively with providers, clinical teams, and insurance plans
• Passion for improving patient outcomes and access to quality healthcare
• Bilingual English/Spanish strongly preferred
