We are seeking a Home Health Registered Nurse (RN) to provide direct patient care in patients' homes.
Key responsibilities include: assessing patient conditions, developing individualized care plans, administering medications and treatments, educating patients and caregivers, monitoring patient progress, coordinating with healthcare professionals, documenting care activities, ensuring safety and infection control, and supporting families emotionally.
Qualifications required: at least one year of recent RN experience, including six months in home health, proficiency in OASIS documentation, valid RN license, and BLS certification.
Preferred qualifications: home health nursing certification (RN-BC) and additional licenses or certifications.
Skills needed: critical thinking, excellent communication, physical ability to perform duties, interpersonal skills, technical proficiency, and the capacity to work under pressure while maintaining attention to detail.
