- Assess patients' strengths, develop rehabilitation programs aligned with physician plans, and monitor progress.
- Provide clear instructions to PTAs and Home Health Aides, oversee their performance, and ensure accurate documentation.
- Report patient progress to the care team, physicians, patients, and families, submitting all required evaluations and summaries.
- Unrestricted Physical Therapy license and CPR certification.
- Experience collaborating with healthcare professionals; one year of experience preferred.
- Strong communication, assessment, and documentation skills.
