Role: Charge Registered Nurse (RN) responsible for coordinating and delivering clinical care within a PACE center, supporting elderly participants to live independently in the community.

- Provide clinical leadership and support to the care team.
- Manage participants' health conditions in collaboration with providers.
- Administer medications, treatments, immunizations, and wound care.
- Conduct assessments and contribute to care planning.
- Assist with staff onboarding, training, and inventory management.
- Support quality and compliance initiatives, participate in on-call rotations.
- Current RN license, graduate of an accredited nursing program.
- Preferred leadership experience as a Charge Nurse.
- Experience with elderly or long-term care populations.
- CPR certification or ability to obtain.
- Strong clinical judgment, communication, and organizational skills.
- Participant-focused and compassionate approach.
- Willingness to be vaccinated against specified diseases.
- Meet CMS competency standards.
- Flexibility to work beyond normal hours and in varied environments.
