Social Services Director (Skilled Nursing Facility)

Overview:
An established skilled nursing facility is seeking an experienced Social Services Director to oversee and coordinate resident-centered social service programs. The ideal candidate will bring compassion, leadership, and a solid understanding of long-term care regulations to ensure the emotional and psychosocial well-being of residents.
Key Responsibilities:
- Direct and manage all aspects of the social services department.
- Conduct psychosocial assessments and develop individualized care plans.
- Provide counseling and support for residents and families.
- Coordinate discharge planning and collaborate with community resources.
- Work closely with the interdisciplinary team to promote continuity of care.
- Maintain accurate and compliant documentation.
- Supervise and train social services staff.
Qualifications:
- Bachelor’s Degree in Social Work, Psychology, Sociology, or related field (Master’s preferred).
