What are the responsibilities and job description for the Residential Counselor position at Apex Community Care?
Position Summary:
The Residential Counselor provides staff presence and assistance in the small group residence, assists clients in following through with Individual Care Plans, is available to residents and attends to the physical maintenance of the facility during scheduled shift. This Residential Counselor position is full time position that reports to the Housing Director.
Responsibilities:
- Monitor day to day functions of Apex Residential Program
- Support clients in their basic living skills including cooking, cleaning, grocery shopping, transportation, etc.
- Assist clients with implementation of Individual Care Plans
- Maintain shift records according to policy
- Monitor physical maintenance of facility, report facility issues or concerns to Housing Director
- Maintain safe and secure environment
- Facilitate client activities as planned
- Participate in required house and staff meetings
- Provide transportation for residential clients as needed
- Ensure group resident clients attend any evening HIV Support Group
- Oversee company van maintenance and upkeep
- Maintain ongoing communication with supervisor and residential staff regarding day-to-day occurrences at group residence
- Serve as a flexible team member of residential staff; act as generalist to provide other duties as needed
- Maintain client confidentiality
Qualifications:
- High School Diploma Required, lived expertise preferred
- Ability to work some evening shifts
- Must be compassionate and caring individual that is comfortable supporting clients living with HIV and a history of substance use
- Reliability and flexibility
- Valid driver’s license
