Position Summary:
We are seeking a dynamic and compassionate Advanced Practice Registered Nurse (APRN) to join our team. In this vital role, the Nurse Practitioner will provide comprehensive, patient-centered healthcare within their scope of training. The APRN will use evidence-based practice(s) for assessment, diagnosis, treatment and management of acute and chronic conditions. The APRN will practice independently in their day-to-day duties and will be assigned a collaborating physician, if required in accordance with state scope-of-practice laws. The APRN will be focused in primary care, HIV treatment and prevention, hepatitis treatment, STI treatment and prevention. The APRN will participate in team-based, collaborative care planning and will function as an integral part of the whole-health patient team.
Responsibilities:
Provides compassionate, comprehensive, ethical, patient-centered assessment, diagnosis and treatment and management of acute and chronic conditions.
Provides care to those 18 and over; if training and scope-of-practice allow, may provide care to older adolescents.
Provides physical exams and appropriate preventative health measures within prescribed guidelines and healthcare needs of patients
Orders, interprets, and evaluates all necessary lab and diagnostic tests
Based on outcomes of diagnostic tests, labs, and physical assessments, creates in appropriate care plan and follow-up for patients
Evaluates the outcomes of patient care and make any necessary revisions to the care plan
Ensures all pertinent patient care findings are recorded in the patient medical record
Serves as a leader for the medical team at the APRN’s location
Provides appropriate clinical staff supervision
Gives direction to clinical staff appropriate for the staff member’s training and scope-of-practice in accordance with state law(s)
Demonstrates leadership skills in problem solving and communication while overseeing and directing personnel in the medical department
Educates patients, families, and professionals through educational materials, in-service trainings, meetings and publications to assist in their development
Gain thorough understanding of all Apex services with the intent to refer within when necessary, and refer out when appropriate
Work collaboratively with other members of the clinical team such as therapists, case managers, and administrative staff
Consistently participates in the quality assessment and improvement process to help the agency develop and implement improvement plans to enhance patient care, safety initiatives, and risk management
Identifies own learning needs and assumes responsibility for maintaining current knowledge, competency, continuing education, and licensure
Helps articulate and promote the agency’s mission
Performs other duties as assigned, appropriate to training, licensure and in accordance with state and federal law(s)
Required Qualifications:
Degree of MSN with national NP board certification
State of Connecticut licensure (RN & APRN)
NPI
DEA registration and controlled-substance prescriptive authority
BLS
Comfortable with electronic medical records (EHR) systems and computer applications
High integrity and self-motivated
Excellent verbal, written and interpersonal communication skills
Expert clinical judgment and ability to be self-directive and demonstrate initiative
Preferred Qualifications
Experience with HIV/AIDS diagnosis and treatment
Experience with HIV prevention and post-exposure treatment
Experience with STI prevention and treatment
Experience with viral hepatitis diagnosis and treatment
Certification in HIV Medicine
Able to appropriately address the health needs of LGBTQ patients
Experience with Telehealth
Bi-lingual skills English/Spanish

Pay: $50,000.00 - $80,000.00 per year
