Join our dynamic healthcare team as a Home Care Nurse Supervisor (LPN / RN) in a flexible part-time role.
Responsibilities
Core Responsibilities:
- Client Intake & Assessments: Travel locally to clients' homes to perform initial intakes, physical/cognitive service reviews, and complete state compliance paperwork.
- Care Plan Development: Design customized, Person-Centered Service Plans tailored to each client's specific daily personal care needs and state-authorized hours.
- Caregiver Interviews & Vetting: Conduct professional interviews and execute mandatory state competency checklists to evaluate incoming Direct Care Attendants.
- Strategic Caregiver Matching: Intentionally assign and schedule the ideal caregiver to each client's home based on personality, availability, and care requirements.
- Quality Assurance Audits: Conduct routine supervisory follow-up visits (every 60–90 days) to review active charts, check EVV records, and update service plans.
- Case Manager Networking: Eventally act as our primary local point of contact with Montana DPHHS Senior and Long-Term Care case managers to receive, review, and accept incoming client referrals.

Pay: $38.00 - $45.00 per hour
