Primary Responsibility:
The Intake Coordinator at APC Home Health Services is responsible for facilitating the communication functions of the business office and providing clerical support. This role ensures that all cases are received, entered into the system, tracked to meet deadlines, and submitted promptly to the appropriate programs as directed by the Supervisor, supporting APC’s mission to provide high-quality, compassionate home healthcare services to our clients.

Duties and Responsibilities:

• Receive and appropriately coordinate all referrals for APC Home Health Services.
• Enter cases into the computer system and maintain an organized tracking system to ensure all time frames are met.
• Submit paperwork promptly to the appropriate programs following Supervisor instructions.
• Follow up with the proper payee sources regarding client cases while maintaining accurate and thorough documentation.
• Review office reports and take correct action as required.
• Perform clerical duties to support the overall operations of the homecare agency.
• Provide support to the business office and other staff as needed.
• Perform additional duties as assigned by the Supervisor.

• Proficiency with office software and tracking systems.
• Ability to work independently and collaboratively in a team environment.
