Overview
Provide client supervision and conduct activities in accordance with professional staff direction and unit procedures. Assist clients in activities of daily living, encourage client participation in daily scheduled activities, maintain safe and clean environment. Document service activities in accordance with unit procedures. Assist with development and implementation of program schedule of activities.
Essential/Core Job-specific Duties And Responsibilities
- Plans, coordinates, and conducts psychosocial rehabilitation groups using Agency approved modules and according to the unit schedule. Conducts individual PSR sessions as dictated by the client service plan.
- Completes individualized documentation of all psychosocial rehabilitation activities in accordance with the Client Record Handbook and the client service plan.
- Participates in or assists case manager with development and update of rehab services component of client service plan (CSP).
