About AOC
AOC Outreach Services is a Texas-based mental and behavioral health agency serving children, adolescents, and families through an in-home, community-focused model. We deliver Mental Health Targeted Case Management (MHTCM) and Mental Health Rehabilitative (MH Rehab) services aligned to Texas Administrative Code and the Texas Resilience & Recovery framework.
Role Summary
As a Community Services Specialist (CSSP), you will deliver community-based MHTCM and MH Rehab services that help clients build skills, improve daily functioning, and access needed medical, social, educational, and behavioral supports. This is a field-based, client-facing role with flexible scheduling and bi-weekly supervision support.
What You’ll Do
- Provide age-appropriate, individualized MH Rehabilitative services to address functional impairments affecting community integration and daily living.
- Deliver MHTCM activities: comprehensive assessments/reassessments, person-centered plan of care development, referrals/linkage, and ongoing monitoring/follow-up.
- Coordinate care with providers, schools, and community resources to reduce barriers and improve outcomes.
- Complete timely, accurate clinical documentation supporting medical necessity, billing, and TRR/Medicaid requirements.
- Conduct initial outreach within 48 hours of caseload assignment and maintain contact per client Level of Care (LOC) and ISP timelines.
- Collaborate in bi-weekly clinical supervision and with a Field Supervisor for practice support, training, and productivity strategies.
Qualifications (CSSP Pathway)
- Required (CSSP for MHTCM eligibility):
- High school diploma; and
- Three (3) continuous years of documented full-time experience in mental health rehabilitative services prior to August 30, 2004; and
- Demonstrated competency in the provision and documentation of MHTCM and MH Rehab services.
- Strong written documentation skills and familiarity with Texas Medicaid/TRR guidelines preferred.
- Reliable transportation; willingness to travel to client homes/community settings.
- Professionalism, cultural humility, and commitment to trauma-informed, person-centered care.
Compensation & Structure
- Status: 1099 Independent Contractor (not an employee; no company benefits).
- Pay: Hourly rate for eligible, authorized MHTCM and MH Rehab services (enter range).
- Training/Supervision Pay: Eligible AOC CECs and clinical supervision compensated at $18.00/hour.
- Payment: Bi-weekly via direct deposit upon invoice and documentation approval.
- Caseload: AOC aims to assign 2–5 clients within 30 days of contract execution, subject to availability and clinical appropriateness.
Support You’ll Receive
- Client onboarding managed by AOC (insurance verification, consent, initial evaluation, CANS, initial ISP).
- Assigned licensed clinician for bi-weekly supervision and an assigned Field Supervisor for day-to-day support.
- Access to AOC training and continuing education opportunities.
Schedule
- Flexible, community-based schedule set around client needs (some evenings/weekends may be required).
