Position Summary:
ANX Home Healthcare and Hospice Care offers a flex program for Newly Licensed Registered Nurses (NLRN). The 16-week RN transition program will give you the skills to succeed as a home health or hospice nurse. We offer a comprehensive orientation program along with hands-on patient care under the expert guidance and supervision of a senior RN.
- Plan, organize and direct home health care services, from initial evaluation and assessment, pain and medication management, and care plan development.
- Ensure high quality patient experience and health outcomes through a comprehensive and personalized plan of care, proactively making revisions based on patient needs.
- Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA), and collaborate with the patient's primary care physician to ensure continuity of care.
- Provide coaching and education to patients and their caregivers based on clinical diagnosis, empowering them to self-manage their condition, prevent complications, and improve quality of life
- Positions available in San Francisco, San Mateo, and Santa Clara County
Minimum Qualifications:
- Current CA Registered Nurse (RN) license
- Current CPR card for Healthcare Providers
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
- Demonstrates excellent observation, problem-solving, verbal and written communication skills; nursing skills per competency checklist.
Salary : $50 - $58
