Job Summary
We are seeking a dedicated and compassionate Part-time Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team for morning shifts 15 hours per week . The ideal candidate will provide high-quality patient care within our youth group home and outpatient locations along with a strong commitment to patient-centered care. The successful applicant will utilize their expertise in medical documentation, patient assessment, and clinical procedures to support our mission of delivering exceptional healthcare services.
Duties
- Conduct comprehensive patient assessments, and health histories using EMR/EHR systems
- Administer medications
- Support discharge planning, case management, and coordination of or home & community care services.
- Ensure compliance with HIPAA regulations and maintain accurate medical documentation of patient care activities.
Experience
- Active nursing license: Must be licensed as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the State of Ohio with no restrictions.
- Clinical healthcare knowledge: Experience with healthcare regulations, patient care documentation, medication monitoring, and basic clinical nursing tasks in a healthcare setting.
- Communication and professional skills: Demonstrated excellent oral and written communication skills, strong time management, and ability to work collaboratively within a healthcare team.

Pay: $24.00 - $29.00 per hour
