The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y.
Jordan Health is now seeking a Lead Community Health Worker who, under the guidance of the Director of Care Coordination, is primarily responsible for providing oversight of training and guidance to Community Health Workers.

Requirements
The Lead Community Health Worker will ensure to:
· Improve performance and effectiveness of Community Health Workers with patients.
· Establishes and maintains successful collaborative relationship between Community Health Workers team and other departments.
· Establishes and maintains successful partnerships with external agencies.
· Perform all duties of a Community Health Worker with own small caseload.
· Supervise and complete bi-weekly timecards for all Community Health Workers.
Education And Experience Required:
· Associates degree in human services or commensurate experience in outreach/community health.
· Medical office/hospital or outreach experience a plus.
· Experience with diverse cultures and populations preferred.
· Demonstration of supervisory and or leadership experience preferred.
Licenses and Certifications:
· Must have a valid driver’s license and reliable transportation.
· BLS through the American Heart Association
· NYS Infection Control certification
