Job Objective:
Responsible for Intake area processing. Assists in all office and clerical needs, as assigned.
Duties and Responsibilities:
- Intake
- Responsible for communication between disciplines, patients, physicians, etc.
- Assists with clinical scheduling process.
- Facilitates requests for Durable Medical Equipment and/or supplies.
- Responsible for coordination of Referral/Admission process.
- Maintains current patient roster.
- Process/track ongoing authorization requests.
- Assist with auditing of patient charts.
- Provides on-going support to field staff.
- Customer Service
- Develops relationships with referral sources, physicians and outside vendors.
- Primary contact for home health patients.
- Performs patient satisfaction follow-up phone calls.
- Represents department at designated community events in coordination with AVH Marketing department.
Non-Essential Duties:
- Faxing/Photocopy and other clerical duties as it relates to the position.
- Participates in agency and hospital committees as appropriate.
- Participates in orientation and education of other personnel.
- Participates in Quality Improvement (QAPI) process and PI committees.
- Practices excellent phone etiquette when representing AV Home Care.
- Other clerical duties as assigned and as it relates to position.

- High School Diploma
Experience
- At least 1 year recent relevant secretarial experience in an acute hospital or clinic setting.
- Home Health experience preferred.
Required Licensure and/or Certifications:
- Maintains a valid California driver’s license and evidence of current auto insurance.
