- Primarily: Menlo Park VA Medical Center, Community Living Center, 795 Willow Road, Menlo Park, CA 94025
- Palo Alto VA Medical Center, 3801 Miranda Avenue, Palo Alto, CA 94304
Responsibilities
- Provide full-scope geriatric nursing care in Community Living Centers
- Deliver patient care that meets or exceeds nationally recognized standards
- Perform all RN-level clinical tasks required for geriatric patients
- Carry out nursing care plans and document care accurately in VA systems
- Monitor patient status and respond to changes in condition
- Collaborate with interdisciplinary teams to ensure continuity of care
- Maintain compliance with infection control, safety, and facility protocols
- Sign in/out daily; follow COR timekeeping and documentation procedures
- Use all VA-provided clinical equipment, technology, and supplies
- Report exposures, incidents, or safety concerns per VA policy
- Holds a diploma, Associate Degree or Bachelor of Science in Nursing from an accredited program, college, or university and hold a current license to practice as a Registered Nurse in any State; a Bachelor’s degree is preferred
- Has been practicing actively as an RN in an acute, long-term care setting, inpatient/SNF/geriatric setting for at least the last two years
- Is capable of providing complex and comprehensive nursing care for adult medical and surgical patients
- Has and maintains current Basic Life Support (BLS) certification
- Has no health or physical disability restrictions that interfere with performance of assigned duties
- Sit, stand, walk, lift, squat, bend, twist, and reach above shoulders during the work shift
- Lift up to 50 lbs from floor to waist
- Lift up to 20 lbs
- Carry up to 40 lbs a reasonable distance
- Push/pull with 30 lbs of force
