Key Responsibilities
- Perform comprehensive patient assessments and evaluations
- Develop and implement individualized therapy treatment plans
- Provide occupational therapy services in the patient’s home
- Document patient progress, outcomes, and medical histories accurately
- Communicate effectively with patients, families, and interdisciplinary team members
- Ensure compliance with all clinical and regulatory standards
- Active Physical Therapist (PT) license in the state of Colorado
- Reliable transportation and valid auto insurance
- Strong clinical judgment and patient-centered care approach
- Excellent communication and documentation skills
- Prior home health experience preferred (training available for qualified candidates)
