Job Description

Nature of Work:

Under the supervision of the Program Coordinator for the Community Case Management (CCM) Program, provide non-clinical, comprehensive case management services to enrolled clients. Services are tailored to meet clients' individual needs while complying with the Drug Treatment Court Program's requirements. The primary goal is to help clients overcome their drug and alcohol use and to encourage them to develop a more productive lifestyle.

Assist individuals with complex substance use disorder (SUD) needs by addressing various aspects of their lives, including medical, mental health, family, employment, criminal justice, housing and financial issues. These areas often pose barriers to achieving success in the recovery process. Facilitate care and streamline transitions with other entities within the county health care system to ensure participants receive appropriate ongoing care. Help individuals access public benefits and other supportive services. Required to complete all necessary documentation for the CCM Program.

The Drug Court is a specialized court docket that addresses the needs of criminal defendants and offenders struggling with alcohol and drug dependency issues. This non-adversarial court team collaborates to assist defendants in becoming productive, law-abiding members of society, using graduated sanctions for non-compliance. The Behavioral Health Administration, through an agreement with the Office of Problem-Solving Courts, supports the Drug Court Program, which employs an individualized, intensive and structured treatment approach.

Deliver targeted education and outreach, both individually and in group settings, covering the entire spectrum of services within the substance use continuum of care. Performance will be reviewed and evaluated by the supervisor to maintain the highest standards of excellence. Responsible for maintaining accurate records in Welligent, a cloud-based Electronic Health Record (EHR) system, documenting all interactions with participants, and ensuring compliance with required training. Responsibilities may include additional tasks as assigned by a supervisor or management.
