What are the responsibilities and job description for the Supports Planner position at Anne Arundel County Department of Aging & Disabilities?
The primary responsibility of this role is to provide assistance and coordination of community services and supports for Medicaid participants residing in the community.
Requirements for this position include:
· A four-year degree in Human Services, Social Work, Nursing, or a related field.
· A minimum of two years' experience working with older adults, and the frail and disabled population.
· Applicants must submit to a background check.
· References are required.
· Previous Supports Planner experience is preferred.
Pay: $27.54 per hour
