What are the responsibilities and job description for the Transition Specialist - On Call position at Ann Storck Center?
The Transition Specialist / Job Coach will work with young adults transitioning into the workforce. The specialist will connect an individual's capabilities and interests to potential jobs within the community and help develop and or improve job skills and career goals.
Areas of Responsibility:
- Maintain a professional image conducive to the business environment representing ASC in a positive
manner.
- Define criteria and assist in identifying adults who will be considered supported employment candidates
and who will follow all Federal, State and Agency guidelines regarding supported employment.
- Perform job analysis and functional assessments of all perspective supported employment position.
- Provide positive job coaching, calling, development and technical adaptive assistance.
- Document all progress with all supported employment candidates including logs regarding business
contracts, placement interviews and employment opportunities.
- Maintain an open communication regarding placement with STEP Staff, family and all concerned
individuals.
- Evaluate and utilizes all necessary tools to identify strengths and weaknesses of all supported
employment candidates.
- Complete monthly report for Program Coordinator and all concerned parties.
- Continue to develop, maintain and explore all new leads to job openings in the community.
- Develop and conduct related supported employment groups with identified adults.
- Maintain necessary documentation regarding Children’s Service Council, etc.
- Advocate employment benefit rights, opportunities, etc.
- Performs all other duties as assigned.
MINIMUM EDUCATION AND WORK EXPERIENCE REQUIREMENTS:
- Associates degree preferred with 1 year of experience in relevant field; Must have a High School diploma or GED from accredited school
- 3 years of Supported employment history including job placement, development, vocational rehabilitation, job coaching and follow up
- Required VALID Florida driver’s license and good driving record.
- Excellent administrative and organizational skills.
- Good business writing skills.
- Highly organized and able to professionally interact with employees and families.
- Computer skills with Google & Microsoft Office
- Must be able to handle stress of working with non-verbal, non-ambulatory clients who need assistance with most aspects of activities of daily living.
- Must be able to deal with offensive behavior, inappropriate or offensive remarks from individuals being served and their families, and staff in an appropriate manner.
- Must be able to separate personal issues and work issues and maintain decorum and self-control in dealing with individuals being served, their families, and other staff.
WORKING CONDITIONS: Primary duties are in a school and business setting with moderate noise levels. Weekends, holidays and overtime may be required.
