Overview
This position oversees planning, development and operations of the Diabetes Education Program at The Lurie Children’s. The CDE promotes and facilitates optimal care and works with a diverse group of individuals including families, physicians, nurses, donors, Foundation Staff, Institutional Review Board, Children’s Hospital of Chicago Research Center staff, Information Management and other hospital staff through clinical, administrative, professional, educational, and quality improvement activities.
Must be willing to travel to Lurie Children’s/Northwestern Medicine Grayslake Outpatient Center location
Responsibilities
Coordinates, facilitates, and performs the education of all patients requiring diabetes education in both the inpatient and outpatient setting consistent with the American Diabetes Association standards, goals, and performance expectations.
2. Coordinates the specific program activities which pertain to the Diabetes patient and include, Clinical (ambulatory and inpatient). Transitions the patient from the Inpatient to the Outpatient setting, Education (both community based and professional education).
3. Documents patient assessment, education, and interventions appropriately.
4. Performs nursing assessments utilizing a variety of assessment methods and/or area-specific tools. Individualizes existing plans of care and develops goals relative to family needs. Evaluates and documents changes to the plan.
5. Performs nursing care according to procedures/protocols. Anticipates impending emergent situations and intervenes effectively.
6. Participates in the development, implementation and ongoing assessment of policies, procedures, and guidelines that define the education of the patient with Diabetes to assure compliance with standards established by Lurie Children’s, JCAHO, American Diabetes Association, and other regulatory agencies.
7. Assess community needs for diabetes education; plans and coordinates cost effective education programs.
8. Establish and maintain rapport among a group of diverse health care providers. Maintain an effective working relationship with these individuals.
9. Provide methods of communication with patients, families and Diabetes Team. Participate in Endocrine and Diabetes division meetings.
10. Provides for professional development through participation in seminars, workshops, professional affiliations and educational programs to keep abreast of latest trends related to diabetes education and management.
11. Participate in research related to the Diabetes Patients.
12. Performs job functions adhering to service principles with customer service focus of innovation, service excellence and teamwork to provide the highest quality care and service to our patients, families, co-workers and others
Qualifications
A BSN degree from an accredited School of Nursing is required. A master’s degree from an accredited school of nursing is preferred. Current licensure as a registered professional nurse in the State of Illinois is required. Certification of Diabetes Education is preferred.
· It is expected that Certification of Diabetes Education will be obtained within one year of employment. Computer skills preferred, especially with Excel and Access.
· A minimum of two years experience in pediatric nursing is required. The ability to physically immobilize children, if necessary and to assess and respond quickly to patient care situations. The ability to effectively communicate with patients/families, physicians, hospital staff, public health staff, social agencies, school personnel, community resources and media. The ability to move equipment and patients.
