What are the responsibilities and job description for the Patient Navigator - Cancer Center position at AnMed?
The Oncology Patient Navigator functions as a central point of contact for the cancer patient from the time of diagnosis across the continuum of care through survivorship or connection to further care. This nurse will service as a care coordinator, educator, and liaison and will identify and close potential gaps in care to provide efficient and coordinated care between departments. The Oncology Patient Navigator provides education on treatment options, emotional support, social support, physical care, communication assistance, and collaborates with community agencies.
Qualifications: SC RN license and BSN required. Previous Oncology experience required. OCN and/or certification in the appropriate field or ability to obtain within two years required. Must be proficient in office computer software. Exceptional communication and customer service skills required.
