SUMMARY:
The Medical Case Manager supports residential clients with medical appointments to ensure compliance with state and agency regulations. Provides assistance to the residential CILAs when needed such as the administration of medication and coverage of first shift when appointments are not being completed for the home.
ESSENTIAL DUTIES:
Maintain case records of medical appointments for Intermittent and 24-hour CILAs, using the agency’s electronic health record system.
Collect the necessary documentation in alignment with the Department of Human Services Rule 115 and 116 for clients and provide the necessary actions to obtain the documentation for file compliance.
Assist the RN-Trainer with monthly medication counts and other pertinent needs related to medication or medical needs for the residents.
Provide onsite residential support with open shift and administer medication to residential clients.
Actively participates in the interdisciplinary team process assisting in the formulation of care plans and treatment objectives.
Coordinates/provides monitoring and evaluation of client medical needs and care, making recommendations for alternative and/or augmentative services as appropriate, with follow- up and documentation of same.
Functions as the primary liaison between each medical practitioner conveying relevant data to ensure timely and appropriate changes in each client's medical care plan; communicating changes to the interdisciplinary team, and all other pertinent parties.
Functions as the primary liaison between the selected health care organizations (hospitals, clinics, home health and visiting nurses, etc.) that provide all routine and emergency care for clients in the programs.
Completes all agency training requirements and secures additional information, education and training as is appropriate to remain abreast of new knowledge, technologies and methodologies.
Ensures adequate understanding of concepts and principles upon which work responsibilities are based and acquires requisite skills to perform job duties effectively and ensure agency services are provided consistently within its mission and objectives.
Maintain NCI, CPR and First Aid Certification. Attend all required meetings and training courses.
Assist with hospitalizations and communication between medical staff and interdisciplinary team members.
Track and monitor appointments and hospitalizations for 24hr and ICILA clients to ensure they are compliant and report discrepancies to Director of Residential, ICILA Manager, Qualified Intellectual Disability Professionals and any other leadership team members that require support.
Assist with HRST assessments or any other assessments requiring medical information of a client.
Transport clients as needed for program support.
Attend specialty appointments to ensure that concerns are addressed.
Assist with transporting and delivery of cycled medications and controlled substances via agency vans.
Communicate with the residential team, in a timely manner, regarding appointments, staff assignments, and ensuring staff are documenting and uploading correctly in Awards.
Performs other related duties as assigned.
REQUIREMENTS:
Must be a certified Direct Support Professional.
Must have at least six years of experience working directly with individuals with developmental and physical disabilities. Associate degree in a medical related field preferred but not required.
Must have at least two years' experience of supporting clients with medical or case management support.
Must be capable of evaluating, planning and decision making to manage routine and unusual resident and program related issues.
Must be able to anticipate patient care and administrative needs.
Must have a valid driver’s license and drive clients using agency vehicles.
Excellent interpersonal and communication skills.
