Job Summary
The Personal Care Provider (PCP) provides non-medical, in-home personal care and supportive services to private-pay clients in accordance with the client's individualized Care Plan and agency policies. The PCP assists clients with activities of daily living while promoting safety, dignity, independence, and quality of life. This position requires professionalism, reliability, respect for client privacy, and accurate documentation of services provided.

Clinical RN Supervisor
Supervision
This position works independently and does not have supervisory responsibilities.
Essential Duties
- Provide non-medical personal care services to private-pay clients as outlined in the individualized Care Plan.
- Assist clients with activities of daily living (ADLs), which may include:
- Bathing, grooming, dressing, and personal hygiene
- Toileting and incontinence care
- Mobility assistance and transfers
- Meal preparation and feeding assistance
- Light housekeeping related to client care
- Companionship and safety supervision
- Promote client comfort, dignity, privacy, and independence at all times.
- Observe and report changes in client condition, behavior, or safety concerns to the agency promptly.
- Follow all agency policies, procedures, and client-specific instructions.
- Complete accurate and timely documentation of services provided using agency-approved methods or software.
- Maintain professional boundaries and confidentiality in accordance with HIPAA and agency standards.
Other Duties
- Participate in orientation, in-service training, and continuing education as requested.
- Assist with quality improvement activities when applicable.
- Perform other related duties as assigned.
Supervisory Responsibilities
- This position has no supervisory responsibilities.
Education and Experience
- High school diploma or equivalent preferred.
- Minimum of six (6) months experience providing personal care or caregiving services preferred.
- Skills validation and/or competency assessment required upon hire.

- Ability to communicate effectively with clients, families, and agency staff.
- Ability to respond professionally to sensitive situations, questions, or concerns.
- Ability to document services accurately and timely.
- Ability to establish and maintain effective working relationships.
- Ability to work independently in a private home setting.
Special Requirements
- Current tuberculosis (TB) screening upon hire and as required thereafter.
- Reliable transportation to and from client homes.
- If driving a personal vehicle for work purposes: valid driver's license and current auto insurance.
- Ability to pass background screening and reference checks as required by the agency.
Physical Requirements
- Ability to regularly sit, stand, walk, bend, stoop, kneel, crouch, reach, push, and pull.
- Ability to frequently use hands for handling, grasping, and feeling.
- Ability to lift and/or move up to 50 pounds frequently and up to 100 pounds occasionally, with or without assistance.
- Ability to provide the level of physical assistance required to safely care for clients.
- Ability to use vision (close, distance, color, peripheral, depth perception) and hearing sufficient to perform job duties.
Work Environment
- Services are provided primarily in private client homes.
- Work environments may vary and include exposure to:
- Changing weather conditions
- Household pets or animals
- Variable lighting, temperature, and air quality
- The environment may occasionally include exposure to wet or humid conditions, cleaning products, or household hazards.
Travel
- Frequent local travel is required to client homes.
