Patient Care Coordinator

Job Summary: The Patient Care Coordinator is responsible for ensuring a smooth and positive experience for patients by coordinating care, managing scheduling, and serving as a liaison between patients, providers, and clinical staff. This role supports efficient clinic operations while maintaining high standards of patient service and confidentiality.

Reports To: Director, Outpatient Services | Manager, Business Office

Hourly Range: $23.00 – $33.40 (hourly rate does not include overtime or bonuses which are not guaranteed)

Essential Job Functions

• Coordinate patient appointments, referrals, and follow-up care
• Serve as the primary point of contact for patients regarding scheduling, care plans, and general inquiries
• Maintain accurate and up-to-date patient records in electronic health record (EHR) systems
• Communicate effectively with providers, nursing staff, and administrative teams to ensure continuity of care
• Verify insurance information and assist with pre-authorizations as needed
• Educate patients on treatment plans, procedures, and next steps in a clear and compassionate manner
• Ensure compliance with HIPAA and organizational policies

Additional Duties

• Assist with front desk coverage and patient check-in/check-out as needed
• Support quality improvement initiatives related to patient experience
• Participate in team meetings and training sessions
• Perform other related duties as assigned

Minimum Requirements

Education:

• High School diploma or equivalent required
• Associate’s degree in healthcare administration or related field preferred

Experience:

• 1–3 years of experience in a healthcare, medical office, or patient services role preferred
• Experience with scheduling systems and EHR software strongly preferred

Physical Requirements

• Ability to sit or stand for extended periods
• Ability to use a computer, telephone, and other office equipment
• Occasional lifting of up to 25 pounds

Working Conditions/Environmental Exposures

Environmental hazards such as chemicals, solvents, needles, needle containers, sharps, blood, body fluids, various infectious/communicable diseases, infectious medical waste, radiation, and/or radioactive materials and/or radioactive waste, proximity to moving mechanical parts.  Protective clothing/equipment may be necessary.

ASH IMMUNIZATIONS AND VACCINATIONS REQUIREMENTS – Aligns with CDC’s Advisory Committee on Immunization Practices (ACIP) for Healthcare Personnel 

• Hepatitis B- Lab evidence of immunity (titer) to Hepatitis B or three doses Hep B vaccine at appropriate intervals
• Influenza- documentation of current seasonal flu vaccine (mandatory from November 1st through March 31st each year)
• Measles, Mumps, and Rubella - Lab evidence of immunity (titer) to Measles, Mumps, and Rubella or 2 doses of MMR Vaccination
• Varicella (chickenpox) - Lab evidence of immunity (titer) to Varicella or 2 doses of Varicella Vaccine
• Tetanus, diphtheria, pertussis (Tdap) - documented proof of immunization
• Tuberculosis Screening - Documentation of a two-step TB skin test within the past year OR lab verification of a negative TB Gold or T-Spot tuberculin test within the past 6 months
