The position is a mix of patient care, care coordination, and medical administrative support. The Nurse Care Manager shall provide primary care consultation, psychiatric care consultation, and work with the BHHO and OHH team, the primary care practice and the member to provide other Section 92 services as necessary, pursuant to the Plan of Care. Specific tasks will include:
- Participation in Supervision to provide consultation on community based medical services, referral processes, and recommendations on prioritizing care as it relates to medical needs.
- Provide consultation on community resources and the process for accessing, examples: Maximus referrals, in home PSS, Homemaker, Nursing services, Pharmacy delivery and prepacking medications services, medical advance directives, assistance devices in the homes.
- Participation in coordination of care meetings to discuss member concerns with PCP offices on a quarterly basis. Supporting the communication and understanding of concerns between the medical personnel and the BHHO care coordinators is an essential task of the Nurse Care Manager.
- Monitor in conjunction with the Care Coordinators the high ED users and support the Care Coordinators in communication with members on the appropriate use of ED vs Primary Care. Ensure documentation of these reviews are completed.
- Periodic review of clients recent PCP visits to review DX’s and medications as a means of supporting the team in identifying patients with high comorbidity and supporting in a higher level of PCP appointments and follow through.
- Provided 1:1 consultation as needs arise on specific patient recommendations.
- Utilize the specific tools that have been provided in assisting the team in developing a comprehensive understanding of the member medically including but not limited to HIN, PCP documentation, VMS information, and past patient records from providers identified by the member through intake.
- Triage phone calls for providers and patients, contact pharmacies and other provider offices to support on the care continuum
- Administration of injectable medications and other miscellaneous nursing tasks.
Pay: $35.00 - $40.00 per hour
