What are the responsibilities and job description for the Care Coordinator position at AngleZ Behavioral Health Services?
The Health Home Coordinator for Members with Serious and Persistent Mental Illness (SPMI) shall draft the Plan of Care for each member, oversee the plan and coordination of services. This roll will also support and encourage members in actively participating in reaching the goals they set together!
The Health Home Coordinator will identify and link member with community resources, facilitate referrals and respond to social service needs. You will collaborate with other team members including doctors, nurses, other care coordinators, and Clinicians. The care coordinator will help our members implement the Plan of Care, monitor and assist them in problem solving potential issues related to the health care system, financial or social barriers.
The candidate must have excellent communication skills, be motivated to make a change in our community and represent the agency and its mission in an engaging fashion.
