Duties
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts when necessary
- Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately while adhering to HIPAA privacy regulations
- Prepare nutritious meals in accordance with individual dietary plans and assist with meal consumption as needed
- Support medication administration following prescribed care plans and document all care activities precisely
- Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed in group homes or home settings
- Engage residents in social activities and provide companionship to foster emotional well-being
- Follow detailed care plans tailored to each individual’s needs, including specialized dementia or Alzheimer's care and working with individuals with autism or other disabilities
Skills
- Proven experience in assisted living, home health, hospice care, or long-term care environments
- Knowledge of patient observation techniques and ability to monitor vital signs effectively
- Certification in CPR and First Aid; experience with emergency response procedures is highly valued
- Familiarity with medication administration, HIPAA compliance, and documentation standards
- Ability to perform heavy lifting safely and operate mobility aids such as Hoyer lifts or resident lifts
- Strong communication skills for working collaboratively with team members, residents, families, and healthcare professionals
- Experience working with individuals with developmental disabilities or dementia-related conditions such as Alzheimer’s disease or memory care needs
