Schedule: Flexible daytime hours (20 hours per week to start) 3/4 Days per week
Job Summary
Angel’s Touch Home Healthcare Services & Adult Day Center is seeking a motivated and professional Outreach Intake Coordinator to help expand our client base and build strong referral partnerships within the community.
This role is ideal for someone who enjoys networking, building relationships, and helping individuals access quality healthcare services. The Outreach Intake Coordinator will visit local healthcare facilities, distribute marketing materials, and educate potential clients and referral partners about our services including Consumer Directed Services (CDS), In-Home Services (IHS), and Adult Day Care.
This position plays a key role in helping families receive support services that allow them to remain safely in their homes and communities.
Key Responsibilities
Community Outreach
- Visit hospitals, rehabilitation centers, senior living communities, dialysis centers, clinics, and community organizations
- Build professional relationships with case managers, social workers, discharge planners, and community partners
- Promote Angel’s Touch services and explain program benefits
Referral Development
- Distribute flyers, brochures, and referral cards
- Follow up with referral sources regularly
- Maintain ongoing relationships with community partners
Client Intake Support
- Call potential clients from lead lists
- Explain services and program eligibility
- Schedule in-home or in-office assessments
- Assist with gathering basic intake information
Adult Day Care Promotion
- Educate families and referral sources about Adult Day Care services
- Promote social activities, transportation services, and caregiver respite benefits
Tracking & Reporting
- Track referral sources and outreach activity
- Provide weekly updates on outreach visits and potential clients
- Maintain organized records of contacts and follow-ups
Qualifications
Preferred:
- Experience in healthcare outreach, marketing, intake coordination, or sales
- Experience in home healthcare, senior services, social services, or medical office environments
Skills:
- Strong communication and interpersonal skills
- Professional appearance and attitude
- Comfortable speaking with medical professionals and families
- Organized and self-motivated
- Ability to build relationships and follow up consistently
Requirements:
- Reliable transportation
- Valid driver’s license
- Ability to travel locally within St. Louis City & County
- Basic computer skills
- Ability to work independently
Compensation
- $18 – $22 per hour (based on experience)
- Performance bonuses available monthly
- Potential for growth into full-time position
