About the position:
The Provider Support Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home & determine the patient’s eligibility for home care services & develop the plan of care to be followed. This Full-Time position will be in a designated area located within a provider where there is a vacant position, increased census, or a need for additional staff.
Qualifications:
- Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
- Current state license as a Registered Nurse (RN).
- One-year experience as a Registered Nurse (RN), two preferred.
- Proof of current CPR and Hepatitis consent/declination.
- Must be willing to travel to all branches within a provider.
- Candidate must be prepared for occasional overnight stays.
- Current state Driver’s License.
- Reliable transportation with valid and current auto liability insurance.
