Responsibilities
- Deliver personalized patient care by assisting with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ conditions through patient observation techniques, accurately documenting vital signs and medical information in electronic health record (EHR) systems
- Assist with medical procedures such as catheter care, medication administration, and wound care while adhering to home healthcare regulations and safety protocols
- Support patients with memory care needs, dementia care, Alzheimer’s care, and end-of-life care with compassion and patience
- Safely transfer patients using appropriate patient transfer methods and equipment like Hoyer lifts or other assistive devices
- Maintain patient confidentiality in compliance with HIPAA regulations while managing medical records and documentation
- Engage in effective communication with clients, families, and healthcare teams to ensure comprehensive patient-centered care
Skills
- Proven experience working with geriatric patients, individuals with developmental disabilities or disabilities requiring specialized caregiving
- Strong knowledge of medical terminology, vital signs measurement, CPR certification, first aid skills, and basic life support procedures
- Familiarity with EMR (Electronic Medical Records) systems and medical documentation practices
- Ability to perform heavy lifting safely and manage physical demands associated with home health or long-term care environments
- Excellent client communication skills combined with compassionate caregiving techniques for dementia or hospice patients
- Experience assisting with ADLs such as cooking, laundry, cleaning, meal preparation, and general home health assistance
- Knowledge of home & community care standards, hospice care protocols, and end-of-life support practices
