Key responsibilities include:
- Completing initial and quarterly assessments as needed.
- Utilizing a personal vehicle to travel to client's homes.
- Documenting the patient's current condition and any observations.
- Completing a structured Non-Medical Home Care Assessment Form to gather detailed information about daily living needs, mobility, and personal preferences.

What We’re Looking For
Required
- Current, active South Carolina nursing license (RN/LPN)
- Valid driver's license and registered/insured vehicle
- Strong assessment, documentation, and communication skills
- Comfort working independently in the community and as part of a collaborative team
Preferred
- Experience in geriatrics, home health, long-term care, or CAP/DA
- Knowledge of community-based resources and services
Job Types: Part-time, PRN, Per diem
Pay: $25.00 - $50.00 per hour
