Job Title: RN FT Case Manager
Location: Plano, TX
Responsibilities:
- Provide comprehensive case management services to patients
- Assess patient needs and develop individualized care plans
- Coordinate and collaborate with healthcare professionals to ensure continuity of care
- Advocate for patients and their families to ensure their needs are met
- Monitor patient progress and make necessary adjustments to the care plan
- Educate patients and their families on healthcare management and resources
- Document patient interactions and maintain accurate records
- Stay up-to-date with healthcare regulations and best practices- Ability to work independently and as part of a team
- Proficiency in electronic medical records systems
Requirements:
- Valid RN license in the state of Utah
- Proven experience in case management or a related field
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills

Pay: $75,000.00 - $85,000.00 per year
